[dm-crypt] _unformatted in /dev/mapper/

Small update. I have rebooted 5 times in a row, everytime UUID of
/dev/mapper/swap is different, I guess it normal. But, 2 times I didn't get
"_unformatted" in /dev/mapper/, so the bug is somewhat unstable.
